How can I create an evaluation template?

Note: You can only create review templates if you have administrator permissions to manage review cycles and custom templates are enabled for your organisation. Can't see the 'Review templates' option? Please contact support@dialog.nl.

A review template determines what employees and managers fill in during an evaluation. A template consists of one or more sections, and each section contains one or more items.

Step 1: Start a new template

  1. Click on 'Management'
  2. Click on 'Review templates'
  3. Click on 'New template'
  4. On the 'Details' tab, enter the name of the template
  5. Enter a description
    The description is optional and is also visible to staff

Step 2: Add a section

  1. Click on the 'Form content' tab
  2. Click on 'Add new section'
  3. Enter the title of the section
  4. Choose an icon for the section
  5. Select who fills in the section: the employee, the manager or both
  6. Click on 'Save'
    The 'Save' button only becomes active once you have entered a title and chosen an icon and a filler.

Step 3: Add items to a section

  1. In the section, click on 'Add item'
  2. Choose the item type*
  3. Fill in the item fields
  4. Click on 'Save'

The fields you see depend on the item type you choose. You can choose from:

  • 'All goals', 'Performance goals' and 'Development goals': automatically filled with the employee's goals.
  • 'Ambitions': automatically filled with the employee's ambitions.
  • 'Competencies': automatically filled with the competencies from the employee's position.
  • 'Core values': automatically filled with your organisation's core values.
  • 'Positions': automatically filled with the employee's position.
  • 'Open question': a question you write yourself.
  • 'Final evaluation': the concluding assessment of the evaluation.

Note: 'Ambitions', 'Competencies', 'Core values' and 'Positions' are only visible if these features are enabled for your organisation.

For each item, you also choose the 'Rating type'. You can immediately see what each rating scale looks like. Don't want a score to be given? Choose 'No score'. You can also add your own placeholder text, which employees and managers see in the input field. If you leave this field empty, we show a default text.

 

Step 4: Preview and save the template

  1. Click on 'Preview' in the top right
  2. Check what the template looks like for the employee and for the manager
  3. Close the preview
  4. Click on 'Save'

You can only save the template once it has a name and contains at least one section with one item.

You have now successfully created a new review template. You can then use the template for an evaluation moment in your review cycle. Please note: you will still need to add the new template to the relevant assessment point yourself; this does not happen automatically. 

 

Good to know

  • Is a template already being used in a review cycle? Then you can no longer edit or delete it. You can still preview or copy it via the 3 dots in the 'Review templates' overview.
  • When you copy a template, the name field is empty. Enter a new name before you save.
  • You can only set one item per template as the final score.
  • Once you have created the template, you will need to add it to the assessment.
